O'Brien didn't get fired for causing Cora's miscarriage, but that's only because they don't know it was her.
Carson didn't get punished for sneaking food to his old vaudeville partner...but that's because it was more cute than wrong. ;)

O'Brien set the soap on the floor intending Cora to slip on it. It was retaliation for Cora seeking a replacement for O'Brien...or so O'Brien thought. In reality, Cora was helping Violet find a replacement for her own lady's maid, who was leaving. O'Brien got an attack of conscience while Cora was still in the bath and started back to the bathroom to get the soap, but it was too late. O'Brien didn't find out that Cora wasn't planning on replacing her until after Cora lost the baby.
