I don't know. It sounded like Courtney had been talking to Ben about skinny dipping earlier. I daresay the producers asked her if she'd actually go through with it and she said yes. They might have facilitated the meeting, but I'm thinking a good portion of the motivation is Courtney's. (She's a MODEL, remember? She's accustomed to posing half nude).
As far as the cameras following Ben, I think they probably follow him EVERYWHERE except the bathroom.

We actually could be deceived with regards to the "skinnydipping". Remember BP2 when Vienna hopped into bed with Kasey?? They blacked out her top, but you could clearly see her straps. She wasn't topless....
